Our Care Manager will visit you in person for an assessment of care in your home or in Hospital or in any residential care facility where the services will be provided .In this meeting our Care Manager and you will work through the services you require from us.
Following the assessment, the Care Manager will; in conjunction with client or their family, develops a care plan individually tailored and responsive to specifically identified needs to be adjusted and approved by Client and his or her family which will then be followed by our caregiver(s).
Based on the developed plan , we will match the best suited caregiver(s) to meet client's needs and preferences. If requested by client we can arrange a meet and greet with the care giver prior to starting the services at NO cost to the client.
Upon selection of caregiver , the services start from the day and time as per your needs. Once you have started services with us, we keep in touch with you to monitor the quality of care to ensure that we perform to the highest standards. We appreciate client’s feedback to keep improving the care. Your feedback will help us to improve and/or change the care plan according to changing needs.
